Background Check Forms

Click HERE to access Chaperone forms.  The bottom two forms must be submitted to Mr. Light after you have completed your background check. If you are a Hamilton County Department of Education employee and have had a recent background check, you will only need to fill out the Volunteer Request Form.

​SPECIAL STATEMENT 

We realize that some families may be experiencing financial hardships and not be able to pay all or any of the Band fees.  We want any student who wants to participate in the band program to be able to, regardless of financial hardships.  If you have a financial need, please contact Mr. Light to work out a solution.  Also, anyone who has the ability and would like to donate towards a needy student's Band Fees may write a check to the SDHS Band Boosters and send it with your child to drop it in the fees box in the Band Room. 
Please note on the check in the memo line that it is for a Band Fee Donation.
