Complete On Line forms before the Mandatory Drop-In and bring completed print forms to the Drop-In. Unless otherwise notified, a notary is present at the Drop-In for your convenience. All forms must be complete BEFORE students come to band camp!
Student/Parent Information Form
Please fill out as much of the form as possible and hit the "Submit" button at the bottom of the page. The information will automatically be transmitted to the band office for processing!
Hamilton County Schools Student/Athlete Release Form
Indemnification Form must be submitted BEFORE a student can participate in any SDHS Band activity. The form must be notarized before it can be accepted.
The Health Form must be submitted to Mr. Light BEFORE your student goes on any band trips. This form gives permission to take your child to the hospital, if needed and permission to administer medications as needed. The form must be notarized before it can be accepted. A copy of the insurance card this student is covered under must accompany the submission of this form. Copies can be made in the band office.
Band Student/Parent Contract
All students and parents must sign the Band Student/Parent Contract. This form agrees to the rules of the band program and provides a professional contract for all students and parents attending band events. Copies can be made in the band office. You only need to submit the last page of this contract!
Financial Commitment Form
All students and parents must sign the Financial Commitment. This form agrees to the Financial Obligation and provides a written contract of agreement. Copies can be made in the band office. Students will not be allowed participation without a signed copy of this document.
